Who these plans are for
Our self-pay physical therapy plans are designed for patients paying out of pocket or whose insurance does not cover the care they need.
No insurance coverage
You're uninsured, or between plans, and paying out of pocket.
Benefits exhausted
You've used up your visit allowance or hit your annual cap for the year.
PT not covered
Your policy simply doesn't include physical therapy services.
Prepaid plan pricing
The more visits you buy, the lower your per-visit rate. Your initial evaluation is charged at the same per-visit rate as your selected plan.
Paying visit by visit? A single physical therapy visit is $160, with an initial evaluation of $200. Prepaid plans offer a lower per-visit rate for patients who expect to need ongoing care.

Select the practice associated with your location, then enter your plan amount in the secure payment portal. Please confirm your selected plan with your clinic before submitting payment.
Payments are processed through a secure, encrypted payment portal. Your payment information is never stored on our website.
